We embrace the Sanctuary Model of Care as a guide for forming and maintaining a Therapeutic Community that promotes Safety and Non-Violence as the basis for the way we help people recover from trauma.

The Community Navigation Specialist have required education and experience and meet all initial and annual training requirement to qualify as a Blended Case Manager (BCM). The Community Navigation Specialist will be assigned a caseload of individuals who are waiting for mental health services (wait-list) and link individuals/families to available community resources/community networks and identified natural resources, by providing education about various human service systems including but not limited to educational needs, medical needs, mental health needs, and intellectual disability needs. The Community Navigation Specialist will utilize the LifeCourse Framework and Tools to develop a LifeCourse portfolio with the individuals assigned to their caseload. The LifeCourse framework is person-centered and utilized to locate and coordinate community-based supports with a focus on supporting individuals so that they can be as independent as possible throughout the lifespan.

Candidates must possess a Bachelor’s Degree in Human Services; requires valid driver’s license and PA Act 33 Child Abuse, PA Act 34 Criminal Records, and FBI Criminal Background Check clearances.
